Long Term Care |
The term "long
term care" is used here to refer to the continuing care of aged or disabled persons
needing more than outpatient care and short-term home care services. Given both the high
cost and preventability of long-term nursing-home placement, both patients and insurers
can benefit from well-crafted Disease Management and Demand Management programming
intended to maintain the independence and family support of these patients. Such
programming can also be used to facilitate short-term nursing home placement with return
to the community after current needs have been addressed.
